General Contact Form

The contact form on this website collects the name, email address, optional telephone number, and a brief message you choose to submit. We use this information to respond to general inquiries. We do not use this form to collect medical records, symptoms, diagnoses, laboratory results, or medication information. The form includes a notice asking visitors not to submit private medical information.

Contact form submissions are delivered to clinic staff through a secured internal inbox. Submissions are retained for 12 months from the date of submission. These general website submissions are not clinical records and are not intended to contain medical information. Airo Inbox does not currently provide automatic deletion at the end of a retention period; the owner must manually review and remove submissions at or after the 12-month mark.

A honeypot field is included to reduce automated spam submissions. No answer data from the Metabolic Health Check-In is transmitted through this form.

Education Early-Access Form

The education page includes an active early-access interest form. This form collects name, email address, optional telephone number, general educational topics of interest, and whether the visitor would also like information about scheduling an initial consultation. It does not collect symptoms, diagnoses, medications, laboratory information, treatment history, or other clinical information. Submissions are tagged "Education Early Access" and delivered to the same secured internal inbox used for general website inquiries. Submissions are retained for 12 months from the date of submission. Airo Inbox does not currently provide automatic deletion at the end of a retention period; the owner must manually review and remove submissions at or after the 12-month mark. Submitting this form does not establish a patient-clinician relationship or schedule an appointment.

Metabolic Health Check-In

This website includes a free, educational Metabolic Health Check-In tool. The check-in is processed entirely within your browser (client-side). Your responses are not transmitted to our servers, not stored in any database, and not sent to Google Analytics or any other third-party service.

The check-in does not collect your name, email address, phone number, date of birth, or any other identifying information. It does not diagnose a condition, determine treatment eligibility, or recommend medication.

We collect anonymous, aggregate analytics events (described below) to understand how the tool is used, but these events do not include your answers, your result pathway, or any combination of data that could identify you.

Google Analytics

This website uses Google Analytics 4 (GA4) to collect anonymous information about how visitors use the site. Google Analytics is loaded only after you accept cookies through the cookie consent banner. If you decline cookies, Google Analytics is not loaded.

Google Analytics data is processed by Google LLC under Google's Privacy Policy and Terms of Service.

Cookies and Local Storage

This website uses a cookie consent banner. If you decline cookies, analytics cookies are not set. If you accept, Google Analytics may set cookies as described above.

The Metabolic Health Check-In uses your browser's local storage to remember whether you have dismissed the check-in invitation or completed the check-in. This information is stored only on your device and is not transmitted to our servers. It is used solely to avoid showing the invitation repeatedly. It does not contain your answers or result.

How We Use Information

We use information collected through this website to respond to general inquiries submitted through the contact form, to understand how visitors use the site through anonymous analytics, and to improve the website. Clinical Information

This website is not a patient portal and does not accept clinical information. This website does not currently offer a subscription service, subscriber login, or online appointment booking. No payment information is collected through this website.

Data Retention

General contact form submissions and Education early-access submissions are retained for 12 months from the date of submission through the clinic's secured internal inbox. These submissions are not clinical records. Airo Inbox does not currently provide automatic deletion; the owner must manually review and remove submissions at or after the 12-month mark.

Google Analytics 4 event-level and user-level data retention is configured in the GA4 property. The owner should confirm that the GA4 property data retention setting is set to two months and that user-level data deletion is enabled. If these settings have not been confirmed, they remain pending owner review.

The Metabolic Health Check-In stores only a dismissal or completion timestamp in your browser's local storage. This information is not transmitted to our servers and is not retained by 423HRT. It expires automatically based on your browser settings or when you clear local storage.

Your Choices

You may decline cookies through the cookie consent banner when you first visit this website. You may also clear your browser's local storage at any time to reset the check-in invitation suppression.
